Individual Services
Visitation Services
Visitation Services- $50/hour. HCPA will visit with your family member or loved ones in their home or nursing facility to expose them to social visits, read, check on their well being. This is a social visit.
Hospital Advocacy
Hospital Visitation- $200/hour. HCPA will provide an in person advocate, function as a Liaison, help ensure quality care and access to the healthcare team. HCPA will provide in depth education on your diagnosis and assist with your plan of care during hospital stay.
End of Life Support
Prevention/Family Support/ End of Life- $100/hour. HCPA will assist with living wills, advance directives, and healthcare proxy. HCPA will make HOSPICE and Palliative Care Arrangements. Please call for Estate Planning details and pricing.
Insurance Assessment/Comparison
Comprehensive Health Care Review- HCPA will assist in reviewing your Current heath care plan and help you select a new plan for your specific healthcare needs. Pricing dependent on number of health plans to compare.
Home Assessments
Home Visit- $150/hour. HCPA will provide an in home assessment of needs, organize and review medications, assist with bill organizing, identify home needs, and review concerns or questions for upcoming doctors appointments and/or healthcare related appointments. HCPA will also attend doctors appointment as requested.
Case Management
Care Transitions/Eldercare/Mental Health and Addiction- $100/hour. HCPA will review your needs prior to transitioning home. HCPA can facilitate placement to a Skilled Nursing Facility, Rehab, Hospital, Home Health, In home Private Care and/or community resources. HCPA will identify support groups, in network providers, and outpatient treatment programs.
Insurance/payor Assistance
Insurance and Payor Assistance- $150/hour. HCPA will review hospital bills, communicate directly with providers and insurers, identify bill errors/discrepancies, Medical bill negotiation. If the Bill is negotiated down we will waive the $150/hour fee and collect anywhere from 10-25% of the amount saved.

Corporate Services
HCPA Corporate program will compliment your existing benefit package at NO expense to the employer. We will give your Employees and their family members access to their very OWN advocate.
